Thanks for the bug report. It appears you have jackd installed, which is
not a normal part of Ubuntu and might steal the sound devices before
pulseaudio starts. Please uninstall jackd, restart, and tell us if the
problem persists.

Also if you have 'timidity' installed please remove that too.
