Hello, I deleted the LUN mappings on the Storage controllers. Once that was done I then used kpartx to delete anything on those two drives. Once that was done I then tried with -av and the mappings got created without any problems. It seems that the information output should be improved on the kpartx tool to provide a more meaningful output in the event that the mappings have been created in the past.
