It seems this was done deliberately some time ago so that cal would be
100% drop-in compatible.  Then in 2020 highlighting was disabled in cal
entirely, upstream in Debian: https://bugs.debian.org/cgi-
bin/bugreport.cgi?bug=904839

Regarding the documentation, it's a little subtle, but the manual page
covers multiple commands, and in the SYNOPSIS section at the top, -h is
not shown as a valid option for cal, and valid for ncal, which seems to
match what each command supports.
