This can happen if the driver is not properly installed. To fully
reinstall the driver please:

1. sudo apt purge '*nvidia*-5*'

2. Reboot.

3. Use the 'Additional Drivers' app to select a new driver.

4. Reboot.

There are also some older bugs with the same symptoms: bug 2063827, bug
2062082, bug 2101817. But you shouldn't experience those if the Nvidia
driver is installed successfully.
