Workaround identified:

The issue can be temporarily resolved by forcing correct display
mapping:

xrandr --output DVI-D-1 --off
xrandr --output HDMI-A-0 --primary --mode 1920x1080

This indicates that the kernel/DRM stack restores an incorrect display
state after resume.

EDID and HDMI link are confirmed to be working.

This is likely an issue in atomic modeset or connector mapping during
resume.
