I'll try on GNOME again, following your suggestion. The way I got it to reproduce there was by following the above steps *very very quickly*, disconnecting and reconnecting the Bluetooth headphones as quickly as possible. After doing this a few times, I got the volume widget to "stick" such that I could turn volume down with the volume keys, but could not turn it up anymore. At that point, I could also use the volume slider in the system menu to mute and unmute the speakers, but couldn't otherwise adjust volume.
