MediaTek MT7921 combo: BT on USB 0489:e0cd, WiFi on PCI 03:00.0 (mt7921e).
After upgrading from 6.8.0-138.138 to 6.8.0-139.139, Bluetooth is completely
unavailable. bluetoothctl lists no controller; bluetoothd starts fine but has
zero adapters. rfkill is unblocked, WiFi is unaffected.
The kernel Oopses during BT firmware setup, so hci0 never finishes
registering:

Reproducible on every boot (4 consecutive boots in the attached journal).
The MediaTek firmware files are present in /lib/firmware/mediatek, so this is
not missing firmware. Booting 6.8.0-138-generic is unaffected.
