Some additional, additional information. The fix I reported above only lasted for a while. Even with the separate /boot partition on my 26.04 it began to fail with the dracut shell appearing on shutdown/reboot again.
I found a post on the Intawebs that mentions molly-guard, which I am running, was getting in the way. I am going to try the fix suggested which was to uninstall molly-guard. Will report back if this solves the issue. One note, it would be nice if the line in the Error; "E: not a regular file:" gave the file it was talking about.
