I have a very similar issue: on boot there is a first request for the
LUKS passphrase, but that fails three times. Then a bit later it asks
again, but now it works

Without splash screen I get the following order

- USB drive with encrypted root partition detected
- requests passphrase to open encrypted partition, but fails (3 times)
- runs /scripts/local-premount
- requests passphrase to open encrypted partition, now succeeds

Kernel:
# uname -rv
7.0.0-38-generic #38-Ubuntu SMP PREEMPT_DYNAMIC Fri Sep 4 09:10:14 UTC 2026
#

Attached is the boot screen to the moment where entering the correct
passphrase would work.
