Public bug reported:

Hello,

On 2026-10-29 at around 14:45 CET, my mouse, USB headphones and
microphone, and built-in keyboard suddenly stopped working at the same
time. The touchpad still worked. Unplugging/reconnecting the USB-C hub
and trying another port didn't help. I used the touchpad to restart, but
the system then appeared completely frozen and I had to hold the power
button to turn it off.

The previous boot's kernel journal recorded this at 14:45:30:

DMAR: DRHD: handling fault status reg 2
DMAR: [DMA Read NO_PASID] Request device [00:14.0] fault addr 0xffd8a000 [fault
reason 0x80] SM: Address in first-level translation is not canonical

System: ASUS ROG Zephyrus G14 GU405AR, BIOS GU405AR.305, Ubuntu 26.04.1 LTS
(x86_64).
Affected kernel package: linux-image-7.0.0-38-generic, version 7.0.0-38.38
(Ubuntu kernel 7.0.14).

00:14.0 is the Intel USB xHCI controller used by the affected USB
devices. The hub reconnects logged at 14:46-14:47 were my recovery
attempts after the failure. I've retained the kernel/shutdown logs and
detailed hardware information as separate files.
