I had the same problem and I solved it by adding option passwdcolumn in
/etc/pam.d/common-password

password    required      pam_mysql.so nullok user=nss-shadow \
  passwd=verysecret db=nss_db table=user usercolumn=user_name \
  passwdcolumn=password crypt=1
