I upgraded my kernel to -12, installed the pulseaudio debugging
packages, and ran gdb. Unfortunately, since pulseaudio wouldn't
terminate, the 'tee' never actually flushed output to the file. I
copy/pasted into a separate buffer. In the attachment you can see the
first invocation and then the second under gdb. I rebooted in between
invocations. Neither would budge under Control-C or Control-\ nor could
I list the process table with ps.

I tried catting various /proc/PID/cmdline files until I found one that
would hang. I then tried 'kill -9' on that PID, but it wouldn't die.

Typing 'ps' under getty made no difference.

The symbol packages I installed:
libpulse0-dbgsym_0.9.9-1ubuntu2_i386.ddeb
libpulse-browse0-dbgsym_0.9.9-1ubuntu2_i386.ddeb
libpulsecore5-dbgsym_0.9.9-1ubuntu2_i386.ddeb
