I've been having this kind of problems too. The problem appears exactly as described above. Very randomly xv mode playback gets heavily distorted (attachment). Running Nvidia restricted driver (nvidia- glx_1.0.9639+2.6.22.4-14.9_i386).
I've noticed that this bug usually appears after a long uptime.
