Thank you! I suspect some sort of race condition (perhaps related to multi-core?), since the failures are quite erratic. Failure at power-up is quite common. Failure during reboot is more frequent during soft reboot (logoff/shutdown/restart button) than during hard reboot (reset button on machine case).
