Now I believe this problem may be related to Nautilus. Nautilus in Hardy has known issues with "Open With...", e.g. see here: https://bugs.launchpad.net/ubuntu/+source/nautilus/+bug/204826
Now after following Permutation's idea above, and removing the second & hidden Bluefish entry under Applications > Other with the Menu Editor, I noticed that Nautilus fortot my .php file "Open With..." association with BlueFish. So it looks like Nautilus by default was using the second ".desktop" command line to try and open my .php file and that explains why it failed 100% of the time, in my default and clean Hardy installation. After doing "Open with another application" on a .php file in Nautilus and selecting Bluefish (and working around the Nautilus bug described here: https://bugs.launchpad.net/ubuntu/+source/nautilus/+bug/204826), now Bluefish opens my .php files properly from Nautilus. Yes per Daniel's comment I have lost the ability to launch .bfproject files from Nautilus with the proper CLI options, but in my case I just don't use the project feature of Bluefish, I just sorely needed to be able to open single files properly.
