Same issue on an installation on amd64.
Installed without lvm. Configured system for working probably, then tried to 
start with lvm. 

Boot fails and falls into command shell. dm_mod is loaded, checked with lsmod. 
The devices for lvm are missing. 
When I do /etc/init.d/udev restart the devices for the lvm appear. So the 
problem might be, that lvm startup is not linked probably with udev startup.
