** Description changed:

Installed from alternate CD using text mode. Didn't create a user
  leaving system unable to login.

Manual partitioning - all in one partition.

+ Update:
This only happens if you forget to format the installation partition. After
warning you when you go back it misses out the create a user step and goes
straight from formatting partitions to installing the base system..
  ------
This happens when you don't format the parition you are installing / to.
After you have written the partition table you get a warning "The target
filesystem contains files from a past installation.. Proceed with installation
to unclean target?". Choose NO. You then get "Not installing to unclean
target", you select GO BACK. You get a warning "Installation step failed.
Failed step - install the base system". You are then presented with the main
menu.

  ...
  Partition disks
  Configure time zone
  Configure the clock
  Set up users and passwords
  Install the base system
  ....

You select partition disks and format the disk. The installation goes
straight to install the base system, it doesn't set up a user.
