I'm encountering the same problem.
Flash component that utilizes your webcam as input will be disallowed even if 
the user said yes to the security question.
This is always reproducible by the following procedures.
1. use a ubuntu machine with webcam.
2. go to http://en.barcodepedia.com/
3. accept the security prompt of the flash component in the left hand side of 
the page.
4. On a windows system, you should see the flash component showing your webcam 
image.
    But on my ubuntu system, the component said "access to webcam disallowed by 
user".

Operating system: ubuntu 8.04.01
firefox: 3.0.1+build1+nobinonly-0ubuntu0.8.04.3
flashplugin-nonfree: 9.0.124.0ubuntu2

I tried to give you the backtrace, however, I failed to follow the instructions 
on https://wiki.ubuntu.com/MozillaTeam/Bugs in the Crash section. 
Firefox-3.0-dbgsym fails to install due to unfulfilled dependencies.
besides, I wonder the backtrace will help since firefox did not crash on such 
occasion.
