Similar case here. I experienced the same when trying to add an
extension - e.g. if I try to add greasemonkey, I click the "add to
firefox..." button on https://addons.mozilla.org/en-
US/firefox/addon/748, I see the dialog appearing in a flash and then
firefox closes.

Like the original submitter, I'm using the KDE4.1 packages; mine are
from from ppa, in particular gtk-qt-engine-kde4 version
1.1+svn142-0ubuntu1~hardy1~ppa2 - I assume his is from ppa as well?

Console output during crash is as follows:

QPixmap: Invalid pixmap parameters
The program 'firefox' received an X Window System error.
This probably reflects a bug in the program.
The error was 'BadAlloc (insufficient resources for operation)'.
  (Details: serial 55740 error_code 11 request_code 53 minor_code 0)
  (Note to programmers: normally, X errors are reported asynchronously;
that is, you will receive the error a while after causing it.
To debug your program, run it with the --sync command line
option to change this behavior. You can then get a meaningful
backtrace from your debugger if you break on the gdk_x_error() function.)

This is a non-debug build obviously.

Just for fun I tried starting firefox with the --sync option; that made
the dialog and main firefox window completely unresponsive, also to
closing the application window.
