Less importantly, instead of appending a not-so-attractive document

cursor while dragging to the cursor theme?

Probably not, because the thing attached to the cursor shouldn't be
generic -- it should look as similar as practical to the thing being
dragged, thumbnailed and/or made semi-transparent if necessary to keep
drop targets visible. So if you're dragging text, that text should be
attached to the cursor (Gedit already does this). If you're dragging an
image, a semi-transparent version of that image (thumbnailed if large)
should be attached to the cursor. If you're dragging a message from the
message list in your mail client, a semi-transparent version of that
row from the list should be attached to the cursor. And so on.
