I am wondering if there is any problem with drive map file created by GRUB. I don't remember exact file name but there should be a .map file in /boot/grub directory. Check if it's contents are appropriate. If they seem fine check if you have any updates pending for grub-pc package. If there are no pending updates try simply running 'sudo update-grub'.
