changelog formatting

needs to contain the specific changelog formatting. From that section it is
rather unclear that you need to only use spaces, to use "-", and to keep the
lines under 80 characters, etc... A full detailed outline would save some
inconsistancies, and would spare people from having to re-fix the same thing.
