I have fixed the problem with swap space by editing the file /etc/crypttab to use the device directly (e.g. /dev/sda4) rather than the UUID (which did not exist under /dev/disk/by-uuid.
While swap space is now operational, suspend and hibernate still fail as reported. Would appreciate advice in addressing this issue.
