I think this is probably related to bug 559070, but we need some more
information to figure out exactly whats going on.
Please can you run the following commands:
sudo slaptest -F /etc/ldap/slapd.d
sudo slaptest -F /etc/ldap/slapd.d -d 1 2>&1 | grep "\.ldif"
sudo fgrep -r "olcAccess" /etc/ldap/slapd.d
Please attach the output to this bug report.
