To assist in further debug, stop the multipath daemon, then run it in the
foreground with max verbosity, while running it under strace. Log strace
to file and the foreground stdout and stderr to another file. Post the results
here.

Also include your multipath.conf, lvm.conf, output of dmsetup table, and
'echo show config | multipathd -k'. 

While the system is in the failure mode, in running ps aux do you see
a series of kpartx processes in uninterruptable state?

I presume you've configured your lvm.conf blacklist correctly? Thanks.
