Same issue here:

Radeon  HD 5450 1GB, 2 Monitors, different sizes.  Changing res,
attempting to setup dual monitors causes  a crash.

Catalyst 8.96.7-120312a-135598C-ATI

Even tried swiping an xorg.conf from a 5870 equipped system with the
desired config, no joy with duals and amdcccle still crashes:
