The second monitor uses an invalid refresh rate, even in lightdm (login
screen).
I have tried 'xrandr --output VGA-0 --auto --left-of HDMI-0 --output HDMI-0
--auto,
however that doesn't help.
When the screensaver is switching from off to on, after switching back to this
computer from another (using a KVM for the second (VGA) monitor and keyboard
and mouse, and a HDMI video switch (doesn't maintain myth of connected display
like KVM does), the second monitor displays the screensaver (i.e. has right
refresh rate), until the second monitor activates.

This is not a problem with a current Debian Wheezy computer on the same
configuration (but different ports on the switches).
