@Andrew: for some reason the radeon kernel module was loaded (and this
prevents fglrx from loading). How did you install the fglrx driver?

Also please try the following:

sudo update-alternatives --set x86_64-linux-gnu_gl_conf 
/usr/lib/fglrx/ld.so.conf
sudo ldconfig -n

sudo update-initramfs -u

Then finally reboot, and attach the new /var/log/gpu-manager.log
