1. It appears you don't yet have the latest version of 'xserver-xorg-
video-intel' that was released in 2016. If you have installed any PPAs
then please remove them using 'ppa-purge' and then fully update the
system:

  sudo apt update
  sudo apt full-upgrade

2. Please also explain in more detail when the problem happens.

3. Your hardware is much newer than the Ubuntu release you are trying to
use so please also try live booting a newer version and tell us if that
avoids the problem:
