That's OK.  If it's a desktop it might not have a make and model,
particularly if it was assembled at your local computer store.  It seems
to have an ASRock motherboard with an AMI BIOS and a Samsung Syncmaster
monitor.  We can be definitely sure that it has an Intel Pentium Dual
Core E2180 CPU running at 2.00GHz, and most importantly it has Intel 945
graphics.

This is occurring after logging in, right?

If you are feeling up to it, you can get more info for us by installing
the latest drm-intel-next mainline kernel from the PPA at
http://kernel.ubuntu.com/~kernel-ppa/mainline/drm-intel-next/ and then
boot and attach the info requested in the How to Get a Batchbuffer Dump
(-intel only) section of the Troubleshooting X freezes, hangs, and
lockups wiki at https://wiki.ubuntu.com/X/Troubleshooting/Freeze
