This bug clearly has nothing to do with xserver-xorg-video-ati, since nVidia and Intel graphics users are reporting the problem as well. I'm chiming in to report that this seems to be affecting VMware installs as well, but only on Kubuntu. It sounds like some sort of race condition, but it hits very often on VMware, which may provide reliable timings for the race condition to occur.
