Are you using any adapters between the card and your Monitor. I had this problem on an S900 with the Ultimate Rez Video Card. I Had an Apple to VGS Monitor Adapter in order to go to a KVM Switch Box. Once I plugged my Apple Monitor directly into the Apple Monitor Port on the Video Card the upper left beachball went away.

I have a 9500 PowerMac w/zForce G3/400 upgrade running OSX v10.1.5 and have to say except for the occasional '<application> has unexpectedly quit' it has been a solid performer. *however* that is until last night. I had the original Mach64 video card in the 9500 and added an ATI Radeon 7000 the other day.. Although I decided *not* to hook up the second monitor, I left the Mach64 in and just used the Radeon. Normally I have not had any problems doing such things, however this has always been in pre-OSX land. Since then, I have had the great displeasure of the beachball on my top-left of the screen right under the beautiful Apple logo. When I look in ProcessViewer, I find that the only thing sticking out is 'Mach-Init' which I assume isn't part of the Mach Kernal, but the Mach64 init portion 'trying' to wake it up. *initially* everything was peachy. The only annoying thing was is when I originally put in the Radeon and powered up the machine ... It showed two displays in the 'Displays' settings, even though there was one monitor was hooked up. I thought removing the video (mach64) would resolve this.. But alas, it did not. So, assuming I am correct and it is the video subsystem trying to initialize the Mach64 -- how do I STOP that? I haven't been able to find a way to 'pull' that out of the startup processes... Any suggestions would be greatly appreciated.
