since what happens is varying from boot to boot, i'd suggest unplugging and reconnecting the monitor, making sure it's fully plugged in. it sounds like the monitor sense pins that tell the card what kind of monitor you have are making intermittent contact. you might also try another monitor, or monitor cable if it's not permanently attached to the monitor.
