This answer is probably better than mine. Some specifics:
install linux,install apache2, install mod-php You can do this on a
very old low end computer if you want, or you can through a hosting
service(xmission) or virtual server provider(linode). Then writing a
simple app like an email form that uses php to save the data to a file
would be a good idea.
