Anything else different on this server? I don't know how the default apache packaging is set up in centos, but could be you've got a conf.d directory with other apache config files that are getting included (e.g. default config for each module). Could be they have some other directives getting mixed in that are wreaking havoc.
403 is interesting ... what are the perms on your doc root directory?
