Create a web enabled screen or screens into which data can be entered.
Screens are printable so that data can be collected on hard copies of the
screens and then entered if necessary; b) create a report that would be
populated from the data entry screens (some calculations of data from the
form into the report); c) create back-end table or tables that the data
populates.



*Preliminary list of data fields:*



1)            Name (first & last)

2)            Address

3)            Phone number

4)            Aliases

5)            Date of Birth

6)            PFN (unique Criminal Identifier)

7)            Docket Number(unique case identifier)

8)            Charges

9)            Reason on Calendar

10)          Active Warrants (yes / no)

11)          Parole hold

12)          Probation violation

13)          Department (courtroom number)

14)          Date of first arrest;

15)          Dates of all previous Failures to Appear in the last 5 years;

16)          Dates of all previous jail incarcerations;

17)          Dates of initial filing (charging document) of any CEN that
includes a Drug Charge code;



18)          Employment information (this needs to be specified more
clearly possibly broken into name of employer, contact name, phone,
address, hours worked);

19)          Residence information (length of time at residence, or
homeless flag).



Some type of verification flag or field may be needed for items 18 & 19.



Note: Items 1 through 17 should be available through a data extract that I
have requested from County ITD: numbers 18 and 19 are probably still
interview based. Its unclear, however, when we can get the extract from
ITD, but two things should determine when we move from Phase 1 and into
Phase 2:



a)            Pretrial Services Staff work with the manual screens for a
couple of weeks and provide input about ease-of-use, functionality, and
present list of potential modifications;

b)            County ITD is ready with the data extract.





*Phase 2:* Integrate data extract into the user form. a) build connection
of some kind and/or routine for importing data from County ITD data extract
into the user form. Again, data elements 1-17, above, should be included in
the data extract. a) Modify form per the input from the Pretrial Services
staff; c) refine back-end to distinguish between i) data elements to be
used for reporting out on Pretrial Services workload (e.g., summary reports
on number of defendants screened, characteristics of defendants, outcomes
of screening), ii) data elements that will be used for case management of
defendants and tracking compliance w/ court orders. Determine if any
additional data entry screens are needed for case management.



*Phase 3:* Refine the back-end. This piece needs to be better defined. Its
not the heart of what were doing, but theres something more here that I
need to think about some more.
