What about adding some GIMP-like toolbar buttons representing the selection mode? These would be mutually exclusive: "add to selection", "replace selection", "subtract from selection".
Thinking again, such buttons should *not* be located in the toolbar of spatial nautilus! This would violate the spatial principles (the selection mode is not a property of the folder). They should be located in an external place (the panel I suppose).
Could get a bit cluttered though... if it's proving to be a problem, it might be easier just to make the default action when you click on an item be "toggle selection state of this item and don't deselect anything else". (Windows already has a mode like this, I forget if gtk+ has or not.)
You are right, the "toggle" solution is much simpler than the "add/replace/subtract" solution. At least, for the limited needs we are talking about. Only one button would be needed.
