Older apps which run under Mac OS 9 (Classic apps), and possibly some Carbon apps (which are not always bundles), do not have the .app extension.
Under Windows, Windows apps should all be .exe, but some older DOS programs may have an extension of .com, and of course batch files may end in .bat or .cmd.
Shell scripts under OS X may end in .sh, .csh, etc....
But the vast majority of "modern" applications will be .exe under Windows, .app under OS X.
