You select a 16 color .ico icon file using Distribution Builder on Windows.
On the Mac a resource file containing the icons will do the trick - have it
include the resource file and it will overwrite the default icons. You may
prefer to do this with ResEdit unless you are building lots of standalones
that require saving a config file for convenience.
