Typically this is indeed result of failed authentication. One other case in 
which I've seen such issue to occur was when there was a proxy between author 
and public instance.

Is this happening to all the users or only to some in particular? Is this 
happening for all the content or only to some? I guess what I'm asking, can you 
see some emerging pattern for occurrence of the problem?
