That'd be nice. Actually it's just a website I found that was ready to go. Getting a survey done in 5 minutes is even more relaxing than doing it with a CouchApp. That said, it'd be really cool to get a survey CouchApp together. Patches welcome! We're thinking of putting up an Apache CouchDB instance where people can showcase their apps.
