You're pretty vague. 

The insert was parallel or sequential? 

The final document counts shows 500.000 docs or 499.994? 

Is it a single instance CouchDB Server? All the docs where in the same
database? 

How was the disk load during the test? 

"No DB Shards could be opened" is a pretty vague message that may be
generated by many different causes. 

If it's randomly appearing on a "normally working" instance, in my
experience it may be related to slow IO: writes to disk pile up and when
the write queue is too long some writes start to timeout giving the No
DB Shards could be opened. 

What where the N and Q settings of your test database?
