Dave,
this is totally right. Neo4j-Spatial right now is in principle
untweaked, and especially the RTree index has no optimizations at all.
We are focusing on getting things right first (just got the
OpenStreetMap shape definitions support Polygons, POI and
Linestrings).

Also note the the RTree is not streaming at the moment, which is
partly a performance hog, and partly going to be a problem with big
data.

Help in optimizing the RTree implementation would be highly appreciated!
