Comment/Referer spam

How is it, with the "simple math question" in place, that spammers are 
still able to enter comments on my site and show up in the referers 
list? Do the spammers have program which can parse the web page and then 
answer those questions? If so, can we swap the math for something else 
like an image with numbers & letters in it?
