Those symptoms and logs are also suspiciously similar to the issue noted in ZOOKEEPER-1748. I have an open PR (https://github.com/apache/zookeeper/pull/83) for it and added a detailed breakdown to the conversation on Github there.
TL;DR: Connection tracking timeouts for the quorum/leader election port may be vexing you.
