Do you see any other exceptions ? I think vhd-util is missing on the XenServer. Download vhd-util @ http://download.cloud.com.s3.amazonaws.com/tools/vhd-util, copy to /opt/xensource/bin and /opt/cloud/bin on XenServer and give executable permissions(a+x).
