Yeah but the Agent State doesn't show so definitely the agent hasn't come up 
which is needed for downloading the iso

Thanks Lennert, I tried also to point the URL to the Ubuntu official download 
link, but still not 
works(http://releases.ubuntu.com/14.04.1/ubuntu-14.04.1-server-amd64.iso). In 
the management-server.log, I did not find clear clues(log file attached), only 
one line:
"There is no secondary storage VM for secondary storage host nfs", but I see 
the SSVM is running there. By the way, is there a way to add the ISO image from 
a local disk .iso file? Thanks!

As long as the state is not ready, it will not show up in the list when you want
to create the instance.  Adding it directly on the secondary storage by yourself
will definitely not work, the system will do this work for you.

Start digging the management servers logs (/var/log/cloudstack/mangement/), to
find out why the ISO isn't correctly added.

Please note that you need to put in a http:// URL when adding the ISO, not a
directory path.
