When logging out of Cloudstack, or if my session expires, it goes into an infinite loop. Actually, it might not be infinite on 4.21 (it shows ~1200 requests), 4.20 it definitely was as it consumed high CPU until you killed the tab. This ONLY happens with SAML auth, and I'm using Okta as my provider. The resolution is to clear all cookies/cache then everything goes back to normal. I've only attempted this on 4.20.1 and 4.21.0 so don't know if this is a regression.
