Another thing to be aware of is disable-output-escaping with Saxon8/9 and Cocoon. Although disable-output-escaping is depricated in XSLT 2 in favour of character maps, if you do use it, it won't work. In the result, the characters that you are attempting to not-escape get wrapped in processing instructions (intended for the serialiser) and remain escaped. Changing the XSLT processor to Saxon does not set Saxon as the serialiser. If anyone knows how to do that, it would be useful info for the Saxon page on the Cocoon Wiki.
Just for reference, I encountered this problem with Saxon 8 and Cocoon 2.1.8 or there abouts.
