I ended up with a similar problem. What was happening was that somehow my first page got defined as a cover page and so it wouldn't show the footer.. and then after that pages for split into sections some of which would show the footer some of which wouldn't.. turned out to be a big mess.
Have you considered redoing the document.. it is just three pages as you say.
